Our referral form can be downloaded below.

It is also available for download in the Accuro Forms Repository by searching "Wascana Physiatry". We will also accept referral letters addressed to us with patient information included. 

We use a pooled referral system by default. This allows patients to be seen by the first available provider. If you would like a specific physician to see your patient, please clearly indicate this in the referral request.
